<b>Hdl-Cholesterol Et Les Autres Facteurs De Risque Cardio-Vasculaires En Milieu Cardiologique A Lome.</b>

Abstract

Objectifs : Déterminer la fréquence des risques cardiovasculaires suivant les taux de HDL-C en milieu cardiologique chez le noir africain, évaluer l’impact du taux de HDLC sur les risques cardiovasculaires.
Malades et méthodes : Il s’agit d’une étude prospective menée sur une période de trois(03) ans, du 1er Aout 2007 au 31 juillet 2010chez les patients reçus en consultation et/ou hospitalisés dans les services de cardiologie de trois(03) formations sanitaires de la ville de Lomé : CHU-Campus ; Hôpital de Bè et le Cabinet Médical "Bonne Espérance".
Résultats : La fréquence du taux de HDL-C bas chez nos patients était de 38,52%. La sex-ratio était de 0,54. L’âge moyen était de 52,31 ± 13,54 ans avec des extrêmes de 18 et 90 ans. Les principaux facteurs de risques associés étaient : l’hypertension artérielle (74,22% des patients avec HDL-C bas et 79,69% des patients avec HDL-C normal) ; l’hypercholestérolémie 51,56% des patients avec HDL-C bas et 55,67% des patients avec HDL-C normal); l’hypertriglycéridémie (25,08% des patients à HDL-C bas et 26,63% des patients à HDL-C normal).Le LDL-C était présent et élevé chez 61,32% des patients à HDL-C bas et 51,13% des patients à HDL-C normal. La cardiopathie ischémique était retrouvée chez 34,49% les patients à HDL-C bas et 06,33% des patients à HDL-C normal.
Conclusion : Le HDL-C tient une place prépondérante dans l’appréciation du risque cardiovasculaire. Un bilan lipidique est nécessaire à partir de l’âge 40 ans. Ceci permettra de corriger d’éventuelles dyslipidémies et de prévenir les maladies cardiovasculaires.

Objectives: To determine the frequency of cardiovascular risk according to HDL-C in cardiac milieu in black Africa, assess the impact of HDL-C on cardiovascular risk and to proposemeasures for prevention of cardiovascular diseases.
Patients and methods: This is a prospective study conducted over a period of three (03) years from 1 August 2007 to 31 July 2010 in three (03) health centers of the city of Lome: CHU-Campus; Hospital Bè and Medical Office "Bonne Espérence"

Results: The frequency of HDL-C patients in our study was 12.80%. 38.52% of patients had HDL-Clow and 61.48% had normal rate HDL-C. The sex ratio was 0.54. The average age was52.31 ± 13.54 years with extremes of 18 and 90 years. Risk factors associated were: hypertension (74.22% of patients with low HDL-C and 79.69% of patients with normal HDL-C), high cholesterol (51.56% of patients with lowHDL- C and 55.67% of patients with normal HDL-C), hypertriglyceridemia (25.08% of patients with low HDL-C and 26.63% of patients with normal HDL-C), hyperglycemia (27 17% of patients with low HDL-C and 18.55% of patients with normal HDL-C), hyperuricemia (25.08% of patients with low HDL-C and 18.12% of patients with HDLC normal). LDL-C was present, and higher on 61.32% of patients with low HDL-C and 51.13% of patients with normal HDL-C. Ischemic heart disease was found in 34.49% patients with low HDL-C and 06.33% of patients with normal HDL-C.

Conclusion: HDL-C plays a major role in the assessment of cardiovascular risk. The assessment of the lipid rate is necessary from the age of 40. This will correct any dyslipidemia and prevention of cardiovascular diseases.
