Affections rhumatologiques observees en hospitalisation a Lome (Togo)

Abstract

Objectif: Déterminer le profil sémiologique et la part respective des différentes affections rhumatologiques motivant une hospitalisation rhumatologique à Lomé.

Patients et Méthodes: Il s’est agi d’une étude transversale menée sur 10 ans et portant sur les dossiers des patients hospitalisés en rhumatologie au CHU-SO de Lomé.

Résultats: 1354 rhumatisants (717 femmes, 637 hommes) ont été hospitalisés dans le service de rhumatologie. Leur âge moyen était de 45,5 ans et la durée moyenne d’évolution de la maladie de trois ans. La durée moyenne d’hospitalisation était de 33 jours. La pathologie dégénérative (562 cas, 41,51 %), la pathologie infectieuse (235 cas, 17,36 %), les rhumatismes inflammatoires et les connectivites (181 cas, 13,37 %) et la pathologie tumorale (109 cas, 8,05 %) ont été les principaux motifs d’hospitalisation. Les principales formes cliniques de la pathologie dégénérative étaient : la lomboradiculalgie par probable conflit discoradiculaire (461 cas), la lombalgie commune (39 cas) et le canal lombaire rétréci (27 cas). Dans la pathologie infectieuse, la localisation était rachidienne chez 165 patients (70,21 %) et extrarachidienne chez les 70 autres (29,79 %). L’infection était d’origine probablement tuberculeuse chez 123 patients et à germes banals chez les 112 autres. La pathologie infectieuse était associée au VIH chez 36 patients. Les connectivites et les rhumatismes inflammatoires étaient représentés principalement par les spondylarthropathies (50 cas) et la polyarthrite rhumatoïde (29 cas). Les tumeurs étaient représentées par 22 cas de myélome et 87 cas de métastases d’origine prostatique chez 24 patients.

Conclusion: L’atteinte dégénérative du rachis, les infections, les rhumatismes inflammatoires et les tumeurs constituent les principales sources d’hospitalisation à Lomé.

English Title: Rheumatological disorders observed in hospital in Lome (Togo)

English Abstract

Aim of the work: Determine the semiological profile and the respective proportion of different rheumatological diseases leading to rheumatologic hospitalization in Lome.

Patients and methods: It was a cross-sectional study conducted over 10 years on the  records of patients hospitalized in rheumatology unit at the Hospital University Centre Sylvanus Olympio of Lome.

Results: 1354 rheumatic’s patients (717 women, 637 men) were hospitalized in the Department of Rheumatology. Their average age was 45.5 years old and the average length of the disease evolution is three years. The average hospital stay was 33 days. Degenerative pathology (562 cases, 41.51%), infectious pathology (235 cases, 17.36%), inflammatory rheumatism and connectivity (181 cases, 13.37%) and tumor pathology (109 cases, 8.05%) were the main reasons for hospitalization. The main clinical forms of degenerative pathology were: the loinradiculalgy by probable disco radicular conflict (461 cases), the common lumbago (39 cases) and the shrunken lumbar canal (27 cases). In Infectious Pathology, the location was spinal in 165 patients (70.21%) and extra spinal in 70 others (29.79%). The infection was probably from tubercular origin in 123 patients and banal germs in 112 others. The infectious pathology was associated with HIV in 36 patients. The connectivity and inflammatory rheumatism were mainly represented by the spondylarthropathies (50 cases) and rheumatoid arthritis (29 cases). The tumors were represented by 22 cases of myeloma and 87 cases of metastasis of prostatic origin in 24 patients.

Conclusion: Degenerative spine, infections, inflammatory rheumatisms and tumors are the main reasons for admission to hospital in Lome.
