Antrolithiase du sinus maxillaire : une pathologie rare

Abstract

L’antrolithiase du sinus maxillaire est une pathologie rare. Elle peut être primitive ou secondaire, se développant sur une pathologie préexistante telle que l’aspergillose.

Nous rapportons le cas d’un patient de 68 ans de sexe masculin venu consulter pour tuméfaction labiale supérieure droite évoluant depuis près de 20 ans, chez qui l’examen physique notait une tuméfaction dure du vestibule buccal supérieur droit. La radiographie standard du crane de face montrait une opacité bien délimitée occupant le sinus maxillaire droit. La chirurgie par voie vestibulaire a permis l’extraction d’une masse de consistance pierreuse. L’examen anatomopathologique a conclu qu’il s’agissait d’une antrolithiase. L’examen mycologique était négatif.

L’antrolithiase doit être retenue comme diagnostic différentiel dans les opacités du sinus maxillaire.

We present the case of a 68 years old male who consulted for a right upper labial swelling which started developing about 20 years ago. His physical examination revealed a hard mass at the right upper buccal vestibule. An anterior-posterior (AP) Xray of the skull revealed a well defined opacity occupying the right maxillary sinus.

Surgery through the vestibule permitted the extraction of the mass which had a stony nature.The pathological studies concluded an antrolithiasis. The mycologic examination was negative. Antrolithiasis has to be retained as a differential diagnosis in maxillary sinus opacity
