Mélanome amélanique de la conjonctive chez une fillette de 11 ans : une observation clinique atypique Amelanic melanoma of the conjunctiva in an 11-year girl: an atypical case report

Abstract

Melanoma of ocular is a rare tumor usually affected patients over 50 years. We report an atypical clinical case of an 11-year-old girl with a left ocular tumor that had been evolving for 2 months. The ophtalmic examination of the left eye noted an upper palpebral mass with chemosis, a collapsed visual acuity (0.1), and an abolished extrinsic oculomotricity. The biomicroscope examination (slit lamp) was difficult to assess on the left. The fundus had returned normal to the right and inaccessible to the left. The histological analysis of the biopsy specimen collected under general anesthesia confirmed the diagnosis of malignant amelanic melanoma of the palpebral conjunctiva. With Adriamycin and Vincristine “Sandwich” chemotherapy spread over 3 months, the evolution was marked by the almost complete regression of the tumor and a total recovery of the visual acuity.

Le mélanome de la conjonctive est une tumeur rare survenant habituellement chez les patients de plus de 50 ans. Nous rapportons une observation clinique atypique d’une fillette âgée de 11 ans ayant présenté une tumeur oculaire gauche évoluant depuis 2 mois. L’examen ophtalmologique à l’œil concerné a montré une masse palpébrale supérieure gauche avec chemosis, une acuité visuelle effondrée (0,1), et l’oculomotricité extrinsèque abolie. L’examen au biomicroscope (lampe à fente) était normal à droite et difficile à apprécier à gauche. Le fond d’œil était revenu normal à droite et inaccessible à gauche. L’examen histopathologique après une biopsie réalisée sous anesthésie générale, a confirmé le diagnostic de mélanome malin amélanique de la conjonctive palpébrale. Sous chimiothérapie en “Sandwich” à base d’Adriamycine et de Vincristine étalée sur 3 mois, l’évolution clinique a été marquée par la régression quasi-totale de la tumeur et une récupération totale de l’acuité visuelle.
