Priapisme induit par la chlorpromazine: A propos de deux cas

Abstract

Le priapisme est une affection rare, l’implication de la chlorpromazine dans l’induction de cette affection est peu connue.

Nous rapportons ici le cas de deux patients qui ont présenté un priapisme en concomitance avec une prescription de chlorpromazine. Devant l’apparition du priapisme en absence de tout antécédent de troubles érectiles et après élimination de toute autre étiologie, le diagnostic d’un priapisme induit par la chlorpromazine a été retenu. Les deux patients ont été traités par ponction-lavage des corps caverneux et injection intracaverneuse d’1-stimulants associées dans le premier cas à la création d’un shunt cavernospongieux. Le priapisme veineux est une urgence urologique. Il constitue un des effets secondaires des neuroleptiques parmi les quels la chlorpromazine. Cet effet iatrogène, qui est rare mais grave, doit être connu par les cliniciens afin d’être mieux prévenu pour éviter les séquelles érectiles.

English Title: Chlorpromazine induced priapism: Two case reports

English Abstract

Priapism is a rare but serious side effect seen with many -adrenergic antagonists, including both typical and atypical antipsychotics.

We report here two cases of priapism associated with chlorpromazine. The first one concerns a young man who was admitted for a manic episode in the context of bipolar disorder. The second one was admitted for behaviour disorders. Upon admission, treatment was begun with chlorpromazine. They presented a priapism. The immediate decision was to stop the antipsychotic agents and to adress both patients in the Urologic Surgery department. They were treated with aspiration and irrigation of the corpora cavernosa with intracavernous injection of sympathomimetic drugs followed by a surgical distal cavernoglanular shunt. This study illustrates two cases of priapism associated with chlorpromazine. It is a urologic emergency. This iatrogenic effect of the chlorpromazine is rare but has to be known by the clinicians to prevent the erectile side-effects.
