Les resultats du traitement chirurgical des fractures de la patella : a propos d’une serie de 23 cas

Abstract

Objectifs: Evaluer les résultats du traitement chirurgical des fractures de la Patella au CHU SO.

Patients et Méthodes: il s’est agi d’une étude rétrospective et prospective allant de janvier 2007 à Décembre 2010 et portant sur 23 cas de fracture de la patella traité chirurgicalement. La série comportait 17 hommes et 6 femmes avec un âge moyen de 35,9 ans. Toutes les fractures ont été classées en trois types selon la classification de Ricard et Moulay. Tous les patients ont été pris en charge chirurgicalement soit par la technique d’embrochage-haubanage (haubanage pre-patellaire) soit par cerclage. Les patients ont été revus avec un recul moyen de 4 ans. A la révision, une radiographie du genou de face et de profil, et une cotation fonctionnelle selon le score de BOSMAN ont été réalisées sur chaque genou traité.

Résultats: un sepsis post opératoire, cinq cas d’ hématome et une raideur du genou ont compliqué les suites à court et à moyen termes. Sur le plan radiologique, un cas de retard de consolidation, et deux pseudarthroses ont été notés. Une arthrose femoro patellaire est apparue à deux ans. A quatre(4) ans de recul moyen, on retrouvait selon le score de Bosman 26,09% d’excellents résultats 65,22% de bons résultats et 8,69% de mauvais résultats.

Conclusion: Le traitement des fractures de la patella doit être exclusivement chirurgical. Le haubanage pré patellaire constitue la technique d’ostéosynthèse de choix dans ce traitement.

English Abstract

Objectives: Evaluating the results of the surgical treatment of patellar fractures at Sylvanus Olympio University Hospital.

Patients and Methods: it was a about a retrospective study going from January 2007 to December 2010 and on 23 cases of patellar fracture treated surgically. The series included 17 men and 6 women with an average age of 35.9 years. All fractures were classified into three types according to the classification of Ricard and Moulay. All patients were surgically treated by the plug-tensioning technique (pre-patellar bracing) or by strapping. Patients were reviewed with a mean of 4 years. At the review, an X-ray and a functional dimensioning based on the Bosman score were performed on each treated knee.

Results: a postoperative sepsis, five cases of hematoma, and one case of stiffness of the knee have complicated the short and medium terms of the postoperative period. Radiographically, a case of delayed bone union and 2 cases of non-union were noted. A femoro patellar osteoarthritis appeared to two years. At four (4) years mean duration of follow-up, there were according to the Bosman score, 26.09% of excellent results, 65.22% of good results and 8.69% bad results.

Conclusion: The treatment of patellar fractures should be exclusively surgical. The pre-patellar guying technique remains the first choice in this treatment.
