Luxation obturatrice de hanche avec enclavement de la tete femorale

Abstract

Les auteurs rapportent un cas de luxation obturatrice post-traumatique de la hanche chez un adulte avec enclavement de la tête dans le foramen obturé. Le diagnostic a été posé après un examen tomodensitométrique avec des coupes frontales et 3D. La réduction orthopédique sous anesthésie générale a été réalisée avec succès. L’évolution au bout d’un an était satisfaisante.

Les auteurs attirent l’attention sur le diagnostic de cette lésion dont l’examen clé est le scanner. Ils mettent l’accent sur les difficultés de réduction d’une telle luxation dont le risque majeur est la fracture du col du fémur lors des manoeuvres de réductions. A partir d’une revue de la littérature, ils déterminent les facteurs pronostiques et proposent une attitude thérapeutique.

English Abstract

The authors report a case of Post Traumatic obturator dislocation of the hip in an adult with isolation of the head in the obturator foramen. The diagnosis was made after a CT scan with coronal and 3D. Closed reduction under general anesthesia was performed successfully. The evolution after one year was satisfactory.

The authors draw attention to the diagnosis of this lesion with the key consideration is the scanner. They emphasize the difficulties of reducing a dislocation such that the major risk is the fracture of the femoral neck during the reduction maneuver. From a literature review, they determine the prognostic factors and offer a therapeutic approach.
