Aspects echocardiographiques et etiologiques des cardiomyopathies chez le noir Africain

Abstract

Buts: Décrire les aspects échocardiographiques et étiologiques des cardiomyopathies chez le noir africain.

Patients et méthodes: Il s’est agit d’une étude prospective réalisée à la clinique cardiologique du CHUCampus de Lomé du 1er janvier 2007 au 31 décembre 2009. Etaient inclus dans l’étude, les patients consultants et hospitalisés, âgés de plus de 18 ans, souffrant de cardiomyopathie confirmée à l’écho doppler cardiaque transthoracique et ayant bénéficié d’un bilan à visée étiologique.

Résultats: Nous avons diagnostiqué 540 cas de cardiomyopathies sur 2720 patients. Le taux de prévalence hospitalière des cardiomyopathies était de 19,85 %. L’âge moyen des patients était de 53 ± 9 ans. Les cardiomyopathies dilatées représentaient 45 % de l’ensemble des cardiomyopathies, les cardiomyopathies hypertrophiques 43,3 %, les cardiomyopathies hypertrophiques dilatées 11,7 %. Les cardiomyopathies hypertensives prédominaient, suivies des cardiomyopathies dilatées d’allure primitive.

Conclusion: Les cardiomyopathies sont des affections fréquentes chez le noir africain, dominées par les cardiomyopathies dilatées. En dehors des cardiomyopathies hypertensives, la recherche étiologique reste difficile.

English Title: Echocardiographic aspects and aetiologies of cardiomyopathies in black african

English Abstract

Aims: To describe the various echocardiographic aspects and aetiologies of cardiomyopathies diagnosed in black african.

Materials and Methods: It was a prospective study of 540 cases of cardiomyopathies diagnosed in the cardiology unit of Lomé University Hospital between january 2007 and december 2009. Patients included in the study were all older than 18 years old. Cardiomyopathy was diagnosed and confirmed by transthoracic echocardiography. Causes of the cardiomyopathy were investigated in all cases.

Results: The diagnosis of cardiomyopathy was made in 540 of 2720 patients evaluated, with a mean age of 53 ± 9 years. The in-hospital prevalence of cardiomyopathies was 19, 85%. Dilated cardiomyopathies was found in 45% of cases, hypertrophic cardiomyopathies in 43.3% and dilated hypertrophic cardiomyopathies in 11.7%. Hypertension was found to be the main cause of cardiomyopathies followed by primary dilated cardiomyopathies.

Conclusion: Cardiomyopathies, especially the dilated form, are frequently diagnosed in Togo. Hypertension is found to be the main cause. Other aetiologies remain difficult to diagnose.
