Le diabete sucre atypique a tendance cetosique: A propos d’une observation a Lome et revue de la litterature

Abstract

Objectif : Décrire les aspects épidémiologiques et cliniques du diabète sucré atypique à tendance cétosique
Méthode : Nous rapportons un cas de diabète sucré atypique à tendance cétosique hospitalisé du 10 au 17 Mars 2014 au CHU SO de Lomé et suivi en consultation externe du 28 Mars 2014 à ce jour.
Observation : Il s’agit d’une femme de 50 ans avec antécédents de diabète familial et d’intervention chirurgicale en 2013 pour fibromyome utérin,
hospitalisée dans le service des urgences médicales du CHU Sylvanus Olympio pour polyurie et polydipsie évoluant depuis une semaine et prostration évoluant depuis 24 heures. L’examen clinique à l’entrée notait une hyperglycémie capillaire à 4,55g/l, une haleine acétonique, une obnubilation et une polypnée de KUSSMAUL. La glucosurie+++ et la cétonurie++ faisaient évoquer une acidocétose diabétique inaugurale sans facteur de décompensation. La patiente a bénéficié d’une réhydratation et d’une insulinothérapie. La phase de rémission a débuté le cinquième jour de son hospitalisation ayant nécessité l’arrêt de l’insulinothérapie. Elle a été libérée sous Metformine. Devant la normalisation de la glycémie les cinq mois suivants, la patiente a interrompu de façon volontaire la prise du médicament. Une surveillance glycémique instituée devant cette
interruption thérapeutique a noté des glycémies variant entre 0,72g/l et 1,05g/l jusqu’au 15 janvier 2017 et des hémoglobines glyquées variant entre 5,7 et 6,1%. Le16 Février 2017, l’hyperglycémie est réapparu (1,27g/l) et s’est accrue au jour le jour jusqu’à 2,77g/l le 25 Avril 2017 avec une hémoglobine glyquée à 7,1%. La patiente a été mise de nouveau sous Metformine. Le gliclazide lui sera associé plus tard jusqu’à ce jour. Conclusion : Lediabète sucré atypique à tendance cétosique est bel et bien présent à Lomé. Des recherches ultérieures sur ce type de diabète s’avèrent nécessaires dans notre pays afin de mieux cerner son épidémiologie et ses aspects cliniques.

English Title: Ketosis prone atypical diabetes mellitus: An observation at Lome and litterature review


Objective: Describe epidemiological and clinical aspects of ketosis prone atypical diabetes mellitus
Method: We report one case of ketosis prone atypical diabetes mellitus hospitalized from 10th to 14thMarch 2014 at Sylvanus Olympio UCH of Lome and follow up externally from 28th March 2014 till now. Observation: It is a 50 years old woman, with a family history of diabetes and a surgical operation for uterine leiomyomas. She was hospitalized in the emergency medical service for polyuria and polydipsia evolving for one week and prostration evolving for 24 hours. Clinical examination on admission has noticed a capillary hyperglycemia (4,55g/l), an acetonic breath, a mental clouding and a polypnea of KUSSMAUL. Tests for sugar and acetone in the urine were positive. The diagnosis of inaugural diabetes ketoacidosis without triggering factors was made. The patient received intravenous rehydration and insulin therapy. The remission started on the fifth day of his hospitalization so that the insulin therapy were stopped. She was prescribed metformin and has been released. The normalization of the glycaemia the five first months, obliged the patient to suspend the treatment. A careful monitoring of glucose control has noted glycaemia ranging from 0,72g/l to 1,05g/l until 15th January 2017 and glycated hemoglobin ranging from 5,7 et 6,1%. On the 16th February 2017, the hyperglycemia reappeared (1,27g/) and was increasing until 2,77g/l on 27th April 2017(Glycated hemoglobin= 7,1%). She was prescribed anew some Metformin and Gliclazide.
Conclusion: Ketosis prone atypical diabetes mellitus is present in Lomé. Further research about this type of diabetes is required in our country.
