Cancer du pancréeas : aspects épidémiologiques et cliniques au CHU-Campus de Lomé

Abstract

But : Analyser les aspects épidémiologiques, cliniques et paracliniques des cancers du Pancréas au CHU Campus de Lomé.
Méthodologie : Il s’agit d’une étude rétrospective réalisée au CHU Campus de Lomé du 1er Janvier 1998 au 31 Décembre 2007, les dossiers des patients retenus comportaient le diagnostic de cancer du pancréas avec les données épidémiologiques, cliniques et paracliniques (échographie et/ou scanner abdominal). Résultats : En 10 ans sur la base de nos critères suscités, 20 cas de cancer du pancréas avaient été diagnostiqués sur 2962 dossiers soit 0,67% des hospitalisations. Le cancer de la tête du pancréas était le plus fréquent avec 17 cas (85%). L’âge moyen des patients était
de 55 ans (extrêmes : 25 ans et 90 ans). La sex-ratio était de 1,5. La consommation d’alcool était l’habitude alimentaire la plus retrouvée. Le tableau clinique était dominé par les douleurs abdominales (75%), l’altération de l’état général (80%) et ictère cutanéo-muqueux (75%).L’échographie abdominale qui était l’examen de première intention avait été réalisée chez tous les malades. Les lésions hypoéchogènes (65%), hétérogène (35%), hépatomégalie (35%), dilatation des voies biliaires (45%) avait été les signes échographiques les plus retrouvés. Aucun des malades n’a bénéficié d’un examen anatomopathologique. Conclusion : Le cancer du pancréas est rare au CHU Campus de Lomé. La tranche d’âge la plus touchée est celle se situant autour de 50 ans. La douleur abdominale, l’ictère cutanéo-muqueux et l’altération de l’état général sont les signes les plus fréquents. L’atteinte de la tête du pancréas a été la plus fréquente.

Objectives: To analyze the epidemiologic, clinical and paraclinic aspects of cancers of the Pancreas in the CHU Campus of Lome. Methodology: It is about a retrospective study carried out with the CHU Campus of Lome from January 1st 1998 to December 31st 2007, the files of the patients selected comprised the diagnosis of cancer of the pancreas with the epidemiologic, clinical and paraclinic data (echography and/or abdominal scanner). Results: In 10 years on the basis of our caused criterion, 20 cases of cancer of the pancreas had been diagnosed on 2962 files i.e. 0.67% of the hospitalizations. The cancer of the head of the pancreas was most frequent with 17 cases (85%). The average age of the patients was 55 years (extreme: 25 and 90 years). The sex-ratio was of 1.5. The consumption of alcohol was the most found food practice. The clinical picture was dominated by the abdominal pains (75%), the deterioration of the state general (80%) and yellow fever skinned-mucous (75%). The abdominal echography which was the examination of first intention had been carried out among all patients. The lesions hypoechogenes (65%), heterogeneous (35%), hepatomegaly (35%), dilation of the bile ducts (45%) had been the most found echographic signs. None patients profited from an anatomo-pathological examination. Conclusion: The cancer of the
pancreas is rare in the CHU Campus of Lome. The age bracket the most touched is that being located around 50 years. The abdominal pain, the skinned-mucous yellow fever and the deterioration of the general state are the most frequent signs. The attack of the head of the pancreas was most frequent.
