Prévalence du carcinome Hépatocellulaire chez les porteurs d’hépatopathie chronique à Yaoundé - Cameroun

Abstract

Introduction: Le carcinome hépatocellulaire (CHC) est la tumeur maligne la plus fréquente du foie. Son incidence a connu une nette augmentation ces dernières années. Dans les régions où les hépatites virales sont endémiques, l'incidence de ce cancer est croissante. Au Cameroun, c'est le quatrième cancer le plus fréquent et la quatrième cause de décès par cancer. L'objectif de cette étude était de déterminer la prévalence du carcinome hépatocellulaire chez les patients porteurs d'hépatopathies chroniques pris en charge à Yaoundé.

Méthodologie: Nous avons mené une étude transversale d'octobre 2013 à mars 2014, avec un volet rétrospectif de 10 mois dans trois grands hôpitaux de Yaoundé. Etaient inclus toutes les personnes à risque de développer un CHC. Etaient exclus les personnes ne présentant pas d'hépatopathie ou n'ayant réalisé aucun examen prouvant leur appartenance au groupe à risque.

Résultats: Au total, nous avons recruté 150 patients dont 59,3% de sexe masculin (n=89) et 40,6% de sexe féminin (n=61). Selon les différentes pathologies, 56% (n=84) avaient une hépatite B chronique, 26,6% (n=40) avaient une hépatite C chronique, 4,6% (n=7) avaient une co-infection BC, 4,6% (n=7) avaient une co-infection BD, 2% (n=3) avaient une cirrhose alcoolique, et 6% (n=9) avaient des masses hépatiques. Parmi les 150 patients, 24,6% (n=37) présentaient un CHC.

Conclusion: la prévalence du CHC chez les porteurs d'hépatopathie chronique reste très élevée dans notre milieu. Il ya lieu de susciter les moyens de prévention et de prise en charge précoce pour le contrôle de cette affection.

English Title: Prevalence of hepatocellular carcinoma in carriers of chronic liver disease in Yaounde - Cameroun

English Abstract

Introduction: Hepatocellular carcinoma (HCC) is the most common malignant liver tumor. The incidence of this cancer has increased markedly in recent years and is rising in areas where viral hepatitis is endemic. In Cameroon, it is the fourthmost common cancer and the fourth leading cause of cancer-related deaths. The primary objective of this study was to determine the prevalence of hepatocellular carcinoma in patients with chronic liver disease being treated in Yaounde.

Methods: A cross-sectional study was conducted from October 2013 to March 2014, including a 10-month retrospective component in three major hospitals in Yaounde. This study included all persons at risk of developing HCC and excluded those without liver disease or who had not undergone any test proving that they were part of the risk group.

Results: In all, 150 patients were recruited, 59.33% of whom were male (n = 89) and 40.66% female (n = 61). Of these patients, 56% (n = 84) suffered from chronic hepatitis B, 26.66% (n = 40) had chronic hepatitis C, 4.6% (n = 7) were co-infected with hepatitis B and C, 4.6% were co-infected with hepatitis B and D, 2% (n = 3) suffered from alcoholic cirrhosis, and 6% (n = 9) had liver masses. Among the 150 patients, 24.67% (n = 37) had HCC.

Conclusion: The prevalence of HCC in carriers of chronic liver disease is still very high in our context. It would be necessary to mobilize resources for prevention and early treatment to control the disease.
