Lumbosacral X-ray abnormalities in patients with chronic low back pain in Bamenda

Abstract

Background: About 11.9% of the world’s population suffers from low back pain (LBP) at any point in time while an over 80% of people will have complained of LBP over a life time. Of all evaluation modalities for chronic low back pain (cLBP), conventional radiography is the most used in our setting, still there is limited data on the radiologic characteristics of cLBP in Cameroon. The objective of this study was to describe the lumbosacral X-ray abnormalities in patients with chronic low back pain in Bamenda
Materials and methods: This was a 2-year descriptive-analytic cross-sectional study at the Bamenda Regional Hospital and Providence polyclinic. We included all adult patients who consulted for chronic low back pain and had done a lumbosacral X-ray while excluding patients with spinal surgery and patient with previously known spinal disease. We collected sociodemographic and clinical data, and participants’ X-ray were interpreted in the presence of a radiologist. Data analysis was done using epi Info 3.5.4. A p<0.05 was considered statistically significant. Results are represented on tables and figures.
Results: A total of 85 participants were retained for this study, 55.30% (n= 47) were female giving a M:F ratio of 1:1.24, age ranged from 21 to 89 years with a mean age of 51.45 years (SD ±14.88). We found up to 87% (n=74) of the X-rays to be abnormal with the three most prevalent abnormalities being osteophytes (71.76%, n=61), facet joint arthrosis (69.44%, n=50), and intervertebral disc degenerative disease (67.03%, n=57). Radicular pain was found in 60% (n=51) of our patients was independently associated with abnormal lumbosacral X-ray (aOR=45.45; 95% CI= 6.19-333.66;). Lumbosacral X-rays could explain the source of pain in 90.54% of our population and 61.17% of X-ray results influenced the management of patients with chronic low back pain.
Conclusion: The frequency of abnormal X-ray amongst patients with chronic low back pain is high. Radicular Pain is positively associated with abnormal Lumbosacral X-ray independent of other factors. Lumbosacral X-ray was taken into consideration in patient management.

Introduction: Environ 11,9% de la population mondiale souffre de lombalgie à un moment donné, tandis que plus de 80% des personnes se plaignent de lombalgie au cours de leur vie. De toutes les modalités de diagnostic de la lombalgie, la radiographie standard est la plus utilisée dans notre contexte mais il y a encore peu de données sur les caractéristiques radiologiques de la lombalgie à Bamenda et au Cameroun. L’objectif de la présente étude était de déterminer les anomalies radiologiques lombo-sacrées chez les patients souffrant de lombalgie chronique à Bamenda à notre connaissance.


Matériel et méthodes: Il s'agissait d'une étude rétrospective d’une durée de 02 ans dans deux hôpitaux de Bamenda. Nous avons inclus tous les patients adultes consultant pour une lombalgie chronique et ayant effectué une radiographie lombo-sacrée tout en excluant les patients ayant subi une chirurgie de la colonne vertébrale, et les patients ayant une maladie vertébrale connue antérieurement. L'analyse des données a été réalisée à l'aide d'Epi Info 3.5.4. Une P<0,05 a été considéré comme statistiquement significatif.


Résultats: Un total de 85 participants ont été retenu pour cette étude. 30 % (n= 47) étaient des femmes, soit un rapport H: F de 1:1,24. L'âge variait de 21 à 89 ans, avec un âge moyen de 51,45 ans (écart-type ±14,88).Nous avons constaté que jusqu'à 87% des radiographies étaient anormales, les trois anomalies les plus fréquentes étant: les ostéophytes (71,76 %, n=61), l'arthrose de l'articulation facette (69,44 %, n=50) et la discopathie dégénérative intervertébraux (77,03 %, n=57). Une douleur radiculaire a été trouvée chez 60% (n=51) de nos patients et ce qui était indépendamment associée à une radiographie lombo-sacrée anormale (aOR=45,45 ; 95% IC= 6,19-333,66 ; P<0,001). Cette étude a également révélé que les radiographies lombo-sacrées jouent un rôle dans l'explication de la source de la douleur chez 90,54% de notre population. Jusqu'à 61,17% des résultats des radiographies lombo- sacrées ont influencé la prise en charge des patients souffrant de lombalgie chronique.


Conclusion: La fréquence des radiographies anormales chez les patients souffrant de lombalgie chronique à Bamenda est élevée. La douleur radiculaire s'est avérée être positivement associée à une radiographie lombosacrée anormale, indépendamment d'autres facteurs. Cette étude a également révélé que les radiographies lombo-sacrées étaient prises en considération dans la gestion des patients.
