Despite its potential to cause irreversible erectile dysfunction, evidence on its clinical management in resource-limited settings remains limited. The aim of this study was to describe the clinical features, treatment strategies, and short-term outcomes of ischemic priapism in SCD patients managed at Laquintinie Hospital, Douala, Cameroon.

Methods: A 5-year cross-sectional retrospective descriptive study was conducted, including 38 male patients with homozygous SCD (SS) admitted for ischemic priapism. Data collected covered demographics, clinical presentation, diagnostic investigations, therapeutic interventions, and immediate outcomes.

Results: The mean age was 16 years (range: 5–28). Most patients presented more than 6 hours after onset, often with severe pain and recurrent episodes. Initial management consisted of aspiration (55%) and surgical shunting (76%). Cavernous blood gas analysis was unavailable. Complete detumescence was achieved in 63.2% of patients, partial in 23.7%, while 13.1% showed no resolution. Preventive SCD therapies, including hydroxyurea and transfusion regimens, were inconsistently applied.

Conclusion: Ischemic priapism in SCD patients in this setting is characterized by delayed presentation, limited diagnostic capacity, and a high reliance on surgical management. Improving early recognition, strengthening community education, and ensuring access to preventive therapies are critical to reducing complications and preserving quality of life.

Introduction: The palatine tonsils and adenoid vegetations are part of Waldeyer’s ring and play an important role in the immune defense of the upper airway, particularly in children.

Introduction: Electroneuromyography (ENMG) is the reference standard for diagnosing and grading carpal tunnel syndrome (CTS), but its accessibility is limited in resource-constrained settings. Our study compared US and ENMG concordance to identify the clinical stages in which ultrasonography can reliably replace ENMG.

Methodology: Forty consecutive patients with clinically diagnosed CTS at the Douala Gynaeco-Obstetrics and Paediatrics Hospital underwent a standardised wrist ultrasound measuring median nerve cross-sectional area and compression signs and a conventional ENMG graded as mild, moderate, or severe. Inclusion criteria were a clinical CTS diagnosis and informed consent; exclusion criteria included prior wrist surgery and other peripheral neuropathies. The protocol received local ethics committee approval, and all examinations were interpreted blinded to the alternative modality. Data were analysed in SPSS v26, and was p<0,05. Results: Forty consecutive patients (70 % women: 45 % housewives, 35 % secretaries) were included. ENMG classified 16 cases as mild CTS, 12 as moderate, and 12 as severe. Ultrasonography demonstrated sensitivities of 100 % for severe CTS, 83 % for moderate CTS, and 44 % for mild CTS.

Discussion: Wrist ultrasonography showed sensitivities of 100 % for severe CTS, 83 % for moderate CTS, and 44 % for mild CTS, with overall specificity above 90 %. These results align with the literature, which reports sensitivities of 77–89 % and specificities of 84–86 % for cross-sectional area thresholds between 10.3 and 12.5 mm². The increase in median nerve cross-sectional area correlated significantly with CTS severity (p < 0.05), reinforcing US reliability in sensorimotor forms. In resource-limited contexts, US being non-invasive and cost-effective can replace ENMG for reliably diagnosing moderate to severe CTS.

Conclusion: Wrist ultrasonography offers excellent sensitivity for moderate to severe CTS (sensorimotor forms) and high overall specificity; however, it remains insufficiently sensitive for mild, sensory-only CTS. In the absence of ENMG, US can serve as a reliable diagnostic tool for moderate to severe CTS based on clinical presentation.

Eric Gueumekane Bila Lamou, Yannick Onana, Noel Edisari Désirée Mbango Ekouta, Annick Mélanie Magnerou, Daniel Massi Gams, Jacques Narcisse Doumbe, Servais AlbertFiacre Bagnaka Eloumou, Calixte Kuate Tegueu, Yacouba Njankouo Mapoure Copyright (c) 2026 https://www.ajol.info/index.php/rmp/article/view/330978 Tue, 28 Jul 2026 00:00:00 +0000 Percutaneous nephrolithotomy in adults: Indications, technique, and outcomes at the Laquintinie Hospital of Douala https://www.ajol.info/index.php/rmp/article/view/330980
Introduction: Percutaneous nephrolithotomy (PCNL) is a minimally invasive surgical procedure for removing renal stones via a transcutaneous access, allowing the use of endoscopic tools to fragment and extract calculi. This study aimed to evaluate the clinical outcomes of PCNL in our setting.

Methodology: We carried out a descriptive cross-sectional study with retrospective data collection from medical records of adult patients who underwent PCNL between June 2018 and June 2025 at Laquintinie Hospital. Data included sociodemographic, clinical, imaging, intraoperative, and postoperative parameters.

Results: A total of 36 patients were analyzed. The mean age was 52.7 ± 13.5 years, with a sex ratio of 0.89. Poor hydration was the most frequent risk factor (86.1%). Renal colic (66.7%) and hematuria (33.3%) were the main symptoms. Right-sided stones predominated (52.8%), with staghorn (47.2%) and pelvic stones (44.4%) being most common. The median stone size was 30 mm. Renal puncture succeeded in most cases, though 13.9% experienced failure due to bleeding, requiring open conversion or delayed reintervention. Surgery lasted 2–3 hours in 52.8% of cases. Nephrostomy drainage was maintained for a median of 2 days, and hospital stay ranged from 3 to 6 days in 80.6% of patients. Complications occurred in 47.2%, mainly fever (27.8%) and hemorrhage (19.4%). Diabetes, significant blood loss, and urinary tract infection were associated with postoperative fever. At one month, 61.1% of patients were stone-free on X-ray, increasing to 80.6% at three months. Renal function remained preserved in all cases.

Conclusion: PCNL demonstrated satisfactory outcomes in our context, offering high stone clearance rates and short hospital stays, with complications generally manageable and mostly classified as Clavien-Dindo grade II.

There are eight phylo-groups of E. coli,
with seven belonging to E. coli sensu stricto (A, B1, B2, C, D, E, F) and one
corresponding to Escherichia clade I. The dissemination of antibiotic resistance
in Escherichia coli is a significant threat to public health worldwide and the
production of Beta-lactamase enzymes is the major cause of E. coli resistance.
ESBLs confer resistance to oxyiminocephalosporins, and often express a
multidrug-resistant phenotype, leaving only limited therapeutic options
Extended-spectrum beta-lactamase-producing Escherichia coli (ESBL-EC). The
lower digestive tract of colonized patients has been recognized as the major
source of ESBL producing organism.
