Cas clinique varicelle maligne de l’adulte jeune revelatrice d’une infection a VIH

Abstract

La varicelle est une infection fréquente, contagieuse, habituellement bénigne chez l’enfant ; cependant, chez l’adulte, elle est rare, sévère et souvent associée à un taux de mortalité élevé. La pneumonie varicelleuse constitue la complication la plus redoutable et la plus fréquente chez l’adulte.

Nous rapportons l’observation d’une patiente adulte jeune immunodéprimée au VIH, sans antécédent de varicelle, ayant présenté une fièvre, un prurit, une polypnée et des éruptions papulo-vésiculo-pustuleuses cutanéo-muqueuses disséminées, évocatrice d’une varicelle maligne. La radiographie pulmonaire a permis de noter un syndrome interstitiel.

Un traitement par acyclovir à fortes doses par voie orale à défaut de la voie parentérale a été institué avec amélioration des signes cliniques, régression des éruptions.

English Title: Malignant chickenpox in young adult revealing HIV infection

English Abstract

The Chickenpox (Varicella) is a common, contagious infection, usually mild in children; however, in adults it is rare, severe and often associated with a high mortality rate. The Varicella pneumonia is the most dreadful and common complication in adults.

We report the case of an immunodeficient HIV young adult patient without history of varicella, who experienced fever, itching, polypnea and disseminated papulo-vesicularpustular mucocutaneous rashes, indicative of a malignant chickenpox. The chest radiography revealed an interstitial syndrome.

A treatment with acyclovir in high doses orally failing the parenteral route was established with improved clinical signs and eruptions regression.
