Abstract

Introduction: l’objectif était de rapporter le cas d’une plaie abdomino-utérine par arme à feu chez une gestante avec mort fœtale.

Observation: Il s’agissait d’une gestante de 33 ans à 34 SA, reçue aux urgences chirurgicales de l’Hôpital de l’Amitié Tchad-Chine le 5 septembre 2023 pour plaie abdominale par arme à feu à la suite d’un braquage.

Résultats: À l’admission, la patiente était stable. La balle était entrée en para ombilicale gauche et sotie en fosse iliaque droite. L’échographie a révélé l’absence des battements cardiaques fœtaux. La radiographie n’a pas objectivé la présence de corps étrangers. La césarienne a permis d’extraire un fœtus mort-né frais portant une plaie d’entrée lombaire gauche et une plaie de sortie scapulaire droite. La patiente était sortie de l’hôpital au 9éme jour postopératoire.

Conclusion: Les plaies abdominales par arme à feu chez la gestante peuvent mettre en jeu le pronostic fœtal. La prise en charge doit être précoce et pluridisciplinaire. 

Introduction: the aim was to report the case of an abdominal gunshot wound in a pregnant woman with fetal death.

Case report: A 33-year-old pregnant woman at 34 weeks' gestation was admitted to the surgical emergency department of the Chad-China Friendship Hospital on September 5, 2023 with an abdominal gunshot wound following a robbery.

Results: On admission, the patient was stable. The bullet had entered the left parumbilical region and exited in the right iliac fossa. Ultrasound revealed the absence of fetal heartbeat. Radiography revealed no foreign bodies. A fresh stillborn fetus with a left lumbar entry wound and a right scapular exit wound was extracted by Caesarean section. The patient was discharged on postoperative day 9.

Conclusion: Abdominal gunshot wounds in pregnant women can be lifethreatening. Treatment must be early and multidisciplinary.
