Evaluation de la gestion des dechets issus des activites de vaccination de routine dans le district sanitaire n° 3 de Lome Commune, Togo

Abstract

Introduction: La gestion des déchets biomédicaux est l'une des priorités sanitaires qui depuis la relance du PEV en 2006 est devenue un problème avec l'augmentation des déchets tranchants et piquants.

Objectif: Evaluer la gestion du système de gestion des déchets issus des activités de vaccination dans le District Sanitaire N° 3 de Lomé-Commune.

Patients et Méthodes: Nous avons réalisé du 1er juillet au 31 Août 2013 dans les 14 formations sanitaires du District Sanitaire N° 5 de Lomé-Commune une étude transversale à visée basée sur l'interview directe à base d’un questionnaire structuré de 53 agents de santé dans les unités de vaccination et d’une observation directe à l’aide d’un guide.

Résultats: Au terme de notre étude, on constate que : un grand nombre de personnel chargé de la gestion (41,3%) n’ont jamais bénéficié d’une formation sur la gestion des déchets biomédicaux en général et vaccinaux en particulier. les étapes de la gestion des déchets vaccinaux ne sont pas respectées dans la plupart des cas. le tri est mal fait dans 70% des cas ; la collecte est réalisée juste à la fin des séances de vaccination dans seulement 30% des cas ; seul un centre sur cinq dispose une salle appropriée pour le stockage des déchets. Tous les centres connaissent le problème de rupture de boîtes de sécurité. Le moyen d’élimination des déchets utilisé est l’incinération. Tous les agents impliqués dans la vaccination ont une connaissance des risques de contamination liés aux déchets vaccinaux.

La gestion des déchets reste un système global dont tous les aspects méritent d’être considérés. Chaque étape de l’élimination est capitale pour réduire les risques Des défaillances au niveau d’une étape peuvent remettre en cause la sécurité de l’ensemble du circuit.

Conclusion: Cette étude montre clairement les insuffisances dans la gestion des déchets dans ces formations sanitaires. Des efforts sont nécessaires pour améliorer la gestion des déchets biomédicaux dans le pays en respectant tous les principes de sécurité des injections ; ceci passe par la formation du personnel soignant et l’assurance de la disponibilité du matériel adéquat dans toutes les unités de soins.

English Title: Assessment of management system of sharp and stingy waste in the vaccination units in the Lome – Commune Health District n°3, Togo

English Abstract

Background: The biomedical waste management is one of the health priorities which since the following up of the PEV has become a problem with increase of sharp and stingy waste.

Objective: To value the management system of the sharp and stingy waste set during four years in the Lomé-Commune Health District N° 5.

Method: We have carried out a transversal study based on health agents in the vaccination units and children healthcare with the help of questionnaires and an observation note to analyse the management supports in order to reach to a description. The study has been carried out the five health training centre of the said district.

Results: In this study the following findings have been noticed A survey carried out on a total of 43 health agents, 14 (33 %) of then have been trained by the PEV and one over 3 in the vaccination units. In all the health training centres, one-usage syringes are used in 99 %.

The stockage means of the sharp and stingy waste after the sorting out is a safe box used in all vaccination units which have never suffered from a lack of stock. Empty serum flasks are used up to 56% of cases as collecting box in the children healthcare units canal bites have been reported during these six last months by surveyed health agents (60%), in most cases in the children health cases in the children healthcare units. A team of the district given responsibility by the District Management is charge of transports and the elimination. The elimination means of safe boxes full used syringes and needles is the incineration at the level the only functional incinerator of Montfort type set at CHU-Campus.

Conclusion: This study show clarity the insufficiency of the management of the sharp and stingy waste must be the priorities for the management system of biomedical waste draw the attention training centres. We must therefore make efforts to improve that management by respecting safety principles of injections and the management steps by ensuring the adequate availability of equipment in all care units.
