Abstract

Une éviscération de l’intestin grêle par voie vaginale survenue au cours d’un avortement clandestin par manoeuvre endo-utérine est une complication rare et grave. Elle détermine une urgence chirurgicale pouvant mettre en jeu le pronostic vital en cas de retard de prise en charge. Les auteurs rapportent un cas avec intestin grêle surtout l’iléon restait piégé dans une brèche du  fond utérin et le segment éviscéré était ischémié et perforé laissant sourdre des selles. Le traitement chirurgical a été, réalisé par voie abdominale et, consistait en une résectionanastomose de l’iléon et curetage de la cavité utérine, puis une hystérorraphie. Le pronostic vital a été préservé ; cependant, le pronostic obstétrical est réservé à cause des séquelles des avortements provoqués notamment des troubles sur la santé de reproduction. 

Vaginal evisceration of the small intestine occurring during clandestine abortion by endouterine maneuver is a rare and serious complication. It determines a surgical emergency that could be life-threatening in the event of delay in treatment. The authors report a case with small intestine especially the ileum remained trapped in a breach in the uterine fundus and the eviscerated segment was ischemia and perforated leaving stools. Surgical treatment was carried out abdominally and consisted of resection-anastomosis of the ileum and curettage of the uterine cavity, then hysterorrhaphy. The vital prognosis was preserved; however, the obstetric prognosis is guarded because of the after-effects of induced abortions, particularly problems with reproductive health.
