Kystes du tractus thyreoglosse: A propos de 56 patients suivis dans le service d’orl pediatrique du Chu de Clocheville (tours - France)

Abstract

Introduction and Objectives: The thyroglossal duct cyst represents the majority of congenital anomalies of the neck with children. The treatment consists of a total exeresis of the lesions. Many techniques have been described but Sistrunk’s procedure is the most widely used. So it became necessary for us to undertake this study in order to evaluate our follow up care.

Material and Method: It is a retrospective study conducted on 56 children monitored from 1994 to 2004 in the Children’s ENT Department of CHU de Clocheville (Tours).
Results: The thyroglossal duct cysts represented 46.66% of the cervical malformations. The average age was 5.22 years with a sex-ratio of 1. In 16.07% of cases, children had presented a history of surgery and 41.37% a history of infections before consultation.
The cysts were revealed by tumefaction in 96.77% cases. In 16.13% an external fistula was noticed. The seat was strictly median in 54.90% cases, left paramedian in 31.37% and right paramedian in 13.72%.
Les lesions were adhyoid in 46.15%, subhyoid in 32.70% and suprahyoid in 21.15%. All the children had an echography.
The operation consisted of an exeresis by the Sistrunk's procedure. Six complications were noticed. After an average monitoring period of 7.35 months, the rate of relapses was 17.85%.
Conclusion: The thyroglossal duct cysts represent the most frequent cervical malformations found with children. Echography is indispensable before any operation. The Sistrunk's procedure is the prime process to minimize the rate of relapses.
