Apport de la mammographie au diagnostic de la pathologie mammaire au Chu tokoin de Lome

Abstract

But : Cette étude de type rétro prospectif a porté sur 314 patientes. Son but était d’évaluer l’activité diagnostique de l’unité de mammographie du CHU Tokoin de Lomé.
Résultats : L’âge moyen des patientes était de 43 ans avec des extrêmes de 15 et 85 ans. Les tranches d’âge de 30 à 40 ans (24,52%), et de 40 à 50 ans (28,03%) étaient les plus représentées. Les nodules (35,03%), la mastodynie (25,80%) et le bilan de dépistage ont été les motifs principaux de l’exploration. Les anomalies mammaires représentaient un taux de 39,17% dont 16,24% de carcinomes mammaires. Les anomalies mammographiques associées prédominaient avec un taux de 40,65% ; alors que les opacités (23,50%) et les surdensités focales (15,45%) n’étaient pas négligeables. A l’échographie, 50,75% des structures hypoéchogènes ont été caractéristiques des lésions classées ACR 5 alors que 100% des lésions circonscrites ont été de grade III ou IV en ACR.
Conclusion : Cette étude a retenu que le fibroadénome prédominait avec 19,43%. La sensibilité de la mammographie était de 71,74% presque égale à celle de l’échographie (71,43%). La spécificité de la mammographie était de 89,65% alors que celle de l’échographie était de 69,70%

Goal: This retro prospective survey concerned 314 women (patients). The goal of this study was to value the diagnostical activity of the mammography unit of the University’s Teaching Hospital of Lome-
Tokoin.
Results: The average age of the patients was 43 years with extremes of 15 and 85 years. The age brackets of 30 to 40 years (24.52%), and 40 to 50 years (28.03%) were most represented. The nodules (35.03%),
the mastodiny (25.80%) was the main pattern brushes of the exploration. The mammary anomalies represented a rate of 39.17% of which 16.24% were mammary carcinomas. The associated mammary abnormalities were most dominant with a rate of 40.65% ; whereas the opacities (23.50%) and the focal over densities (15.45%) were not negligible. At the ultrasound scan, 50.75% of the hypo echoic structures were characteristic of the lesions classified in ACR 5 whereas 100% of the lesions circumscribed were classified in ACR 5 3 or 4.
Conclusion: This survey notice that the fibro-adenoma predominated with 19.43%. The sensitivity of the mammography was 71.74%, nearly equal to the one of the ultrasound scan (71.43%). The specificity of the mammography was of 89.65% whereas the one of the ultrasound scan was 69.70%.
