Réanimation périopératoire d’une duplication digestive à localisation pulmonaire et grêlique.

Abstract

Nous rapportons le cas d’un nourrisson de 7 mois opéré pour séquestration pulmonaire extralobaire droite compliquée dans les suites immédiates à J2 post-opératoire d’une distension abdominale avec rectorragies abondantes. Une exploration chirurgicale réalisée en urgence avait mis en évidence une duplication digestive grêlique. L’analyse anatomopathologique a note une paroi gastrique type cardiale fortement évocatrice d’une duplication oesophagienne ou gastrique pour la pièce de séquestration pulmonaire, et de duplication intestinale pour la pièce de resection digestive. Il s’agit d’une duplication digestive à localisation pulmonaire et intestinale. Le diagnostic a été retardé du fait des signes d’appel peu spécifiques et de la rareté de la pathologie. La découverte au cours de l’acte chirurgical a pour inconvénient une exploration paraclinique préopératoire insuffisante avec une non-maitrise du risque anesthésique pouvant conduire à des accidents peropératoires.


English Summary


We report the case of a 7-month-old infant operated for complicated right extralobar pulmonary sequestration in the immediate aftermath on D2 postoperative of abdominal distension with abundant rectal bleeding. Urgent surgical exploration revealed small intestine duplication. Histological analysis noted a cardiac-type gastric wall strongly suggestive of esophageal or gastric duplication for the pulmonary sequestration specimen, and of intestinal duplication for the digestive resection specimen. It is a digestive duplication with pulmonary and intestinal localization. The diagnosis was delayed due to the non-specific warning signs and the rarity of the pathology. Discovery during surgery has the disadvantage of insufficient preoperative paraclinical exploration with lack of control over the anesthetic risk that can lead to intraoperative accidents.
